COVID-19 Visitor Questionnaire for CGCR
To prevent the spread of COVID-19 and reduce the potential risk of exposure to our volunteers and visitors, we are conducting a simple screening questionnaire. Your participation is important to help us take precautionary measures to protect you and everyone at our spay/neuter clinics and facility.
If the answer is “yes” to any of the questions, access to bringing cats to our clinic will be denied.
